What is TUNA?
TUNA stands for Trans Urethral Needle Ablation. It is a heat-based treatment for an enlarged prostate. An instrument is passed along the urethra and small needles are placed directly into the prostate. Radiofrequency energy is sent through the needles, which heats the prostate tissue. Over the following weeks the treated tissue shrinks back, which makes it easier to pass urine.
How it is done
- The procedure is usually done under sedation rather than a full general anaesthetic.
- A telescope-like instrument is placed into the urethra.
- Small needles deliver heat directly into the prostate, sparing the urethra itself.
- You usually go home the same day, sometimes with a catheter in place for a short time.
The trade-offs
TUNA has one notable advantage: a lower rate of retrograde ejaculation than many other BPH operations. That can matter to some men who want to keep ejaculating normally.
The trade-off is that retreatment is more common than with TURP or laser surgery. Some men have trouble passing urine in the early recovery period and may notice mild blood in the urine.
Important things to know
- The benefit is not immediate. The prostate shrinks gradually over weeks.
- The retreatment rate is higher than with TURP or laser surgery. Some men later need a more definitive procedure.
- Many men have temporary difficulty passing urine in the first days after treatment.
- Mild blood in the urine for a short time is common.
- Newer minimally invasive options like Rezum, iTind, and UroLift have largely replaced TUNA in many practices, but TUNA may still be considered in selected patients.
